Consignor Contract

During your appointment (or before shipment), you will fill out a new consignor contract. The terms and conditions in the contract are listed below: 

 

I, the undersigned, hereby consign the item(s) listed below to Repeat Performance Boutique, located at 4343 Henderson Blvd., Suite 160, Tampa, Florida 33629, for a period of ninety (90) days.

 

Upon the sale of the item(s), I will receive fifty percent (50%) of the final selling price. Checks are issued on the 10th of each month.

I understand that checks must be picked up in person, unless I provide a self-addressed stamped envelope for mailing. I may also choose to have my earnings issued as store credit instead of a check. Checks not cashed within ninety (90) days of issuance will be voided and reissued as store credit only.

I acknowledge that all item(s) brought into Repeat Performance Boutique must be clean and in good condition. Any garment requiring steaming or spot cleaning may be subject to an additional charge.

After the 90-day consignment period, any item(s) not sold or reclaimed become the property of Repeat Performance Boutique, Inc. Unsold items not collected will be donated.

I understand that Repeat Performance Boutique assumes no responsibility for loss or damage to merchandise due to fire, theft, accident, or any other cause.

I understand that there is a $25.00 annual service charge, which will be deducted from my first check.
