Consignor Contract

During your appointment (or before shipment), you will fill out a new consignor contract. The terms and conditions in the contract are listed below: 

I, the undersigned, hereby consign the item(s) below to REPEAT PERFORMANCE boutique, 4343 Henderson Blvd. Suite 160, Tampa, Florida 33629 for 90 days. Upon the sale of the item(s), I will receive 50% of the sale price. Checks are issued on the 10th of each month.

I understand that it is my responsibility to pick up checks, as they are not mailed out. Checks not cashed within 90 days will be voided and reissued for store credit only.

I understand that item(s) brought into REPEAT PERFORMANCE boutique must be clean and in good condition. Any garment requiring steaming/spot removing will be subject to additional charge. After 90 days, any items that are not sold or reclaimed become property of Repeat Performance Boutique, Inc.

I hereby warrant that I have complete title to the merchandise listed and will not hold REPEAT PERFORMANCE boutique responsible for any claim of title to it. I also understand that REPEAT PERFORMANCE boutique assumes no responsibility for loss or damage to merchandise listed by fire, theft, accident or any other cause.

I understand that there is a $15.00 per year service charge, which will be deducted from my first check.

Unsold items not collected after contract expires will be donated.
